Appointment Booking Form

Please read and accept the following terms and conditions before booking your appointment. By proceeding with this booking, I agree to the following:

• I understand that a fully refundable, no-obligation booking deposit of €50 is required to secure my appointment. This will be applied towards treatment costs or refunded in full.

• I acknowledge that the deposit will be forfeited if I miss my appointment or cancel/reschedule with less than 48 hours' notice

• I understand that a 3D CT scan is usually required for dental implant consultations. This is offered at a reduced rate of €50, which can be covered by my deposit. The full price applies if the scan is required for external use.

• If I am booking an appointment with the hygienist, I confirm that I have had a dental check-up within the past 12 months. If not, I understand I must book a check-up first.
